4.15.2 - Time Off Policy
PURPOSE
Time off is available for all contractors to encourage rest, relaxation, and the available time to take care of personal obligations. This time can also be used for contractors to be away from work due to illness or injury (personal and relative), personal requirements, vacation or other reasons as determined necessary by the contractor.
POLICY
Once assigned to a client, contractors may only take the approved amount of time off per year based on the number of years of active service.
GUIDELINES
Time off must be requested with two-weeks’ notice. Failure to request time off with adequate notice could lead to denial of time off request.
Time off cannot exceed the following time periods annually:
Less than 1 year of service – 2 weeks
1-5 years of service – 3 weeks
5+ years of service – 4 weeks
Time off that exceeds the approved amounts could lead to:
Loss of client assignment
Reduction in annual pay increase
In some cases, termination
I understand that my immediate supervisor/agency administrator may require documentation to confirm any illness or injury from a licensed physician when the following conditions exist:
The absence consists of three or more working days in succession for a claimed illness or injury.
There are repeated absences on scheduled weekends to work, absences in connection with scheduled days off, or the one and two-day absences that occur on a repeated basis.
The organization retains the right of requiring evidence of an illness for any absence
